Depression, anxiety, schizophrenia, and substance use disorders collectively account for 13% of the global disease burden but receive less than 2% of health budgets. This founding report examines the scale of the mental health crisis, the structural reasons for its neglect, and the evidence for scalable, cost-effective interventions — including task-sharing models that can dramatically expand treatment access in low- and middle-income countries.
